Care South at Home team recognised for going above and beyond for patients with community award

The Care South at Home team from Bath and North East Somerset has teamed up with the Royal United Hospital and BANES Council to improve the flow of patient care from hospital to home, resulting in an award for best community efforts. The award was presented to the team in recognition of their amazing new ways of working and for going above and beyond for patients.

The Active Recovery Team initiative is an exemplar of integrated working between clinicians, social workers and community providers.
